Semaglutid - první perorální agonista receptorů GLP-1 a jeho postavení v léčbě diabetu

SOUHRN

Od doby uvedení prvního agonisty receptoru pro glukagonu podobný peptid 1 (glucagon-like peptide-1 receptor agonists, GLP-1 RA), který byl schválen v roce 2005 (exenatid 2* denně) pro léčbu diabetes mellitus 2. typu, byly v této lékové skupině vyvinuty nové léky, které mají významný účinek na úpravu glykemie a tělesné hmotnosti. Vývoj podání těchto léků pak vedl k subkutánní aplikaci injekcí 1* denně a 1* týdně. V roce 2021 byl v České republice uveden na trh první perorálně podávaný GLP-1 RA, a to semaglutid. Tento článek poskytuje výsledky z klinického programu PIONEER, které svědčí o účinnosti a dobré toleranci perorálního semaglutidu v léčbě diabetu 2. typu. Článek se také zabývá postavením perorálního semaglutidu v kontextu algoritmu léčby diabetes mellitus 2. typu.

SUMMARY

Since the first glucagon-like peptide 1 receptor agonist (GLP-1 RA) was approved in 2005 (exenatide twice daily) for type 2 diabetes mellitus, the class has developed with newer compounds having more pronounced effects on glycemic control and body weight. Administration of the new medication has been developed with subcutaneous once-daily injections and once-weekly injections. And during the last year 2021 there has been introduced the first oral GLP-1 RA semaglutide to the market in the Czech Republic. This paper provides the results from the program PIONEER suggesting that oral semaglutide is efficacious and well tolerated for glycemic control of type 2 diabetes. The paper follows up its position in the context of the algorithm of type 2 diabetes management.
